Adult Health Home Care Management
Provides case management services to adults with chronic health conditions, serious mental illness, or HIV/AIDS to assist them in staying healthy, out of inpatient care, hospitals, and emergency rooms.
Program Information
Eligibility
Individuals must:
* currently have active Medicaid
* have two or more chronic health conditions, OR be living with HIV/AIDS, OR have a serious mental illness
