Self Direction

Provides an opportunity for participants or their representatives to have decision-making authority over services and take direct responsibility to manage those services, promoting personal choice. This program is Medicaid funded and is OPWDD eligible.

Program Information

Eligibility

Individuals must:
- must be at least 2 years old
- be Medicaid eligible
- have a Medicaid Service Coordinator (MSC)
- go through the Front Door and be OPWDD-eligible
- obtain a start-up broker, a Financial Intermediary (FI), and a Circle of Support (COS)
- have chosen HCBS waiver services over institutional care
