Provides permanent support housing and support for individuals and families with HIV/AIDS.
Provides a drop-in community center for people living with and directly affected by HIV/AIDS. Services include housing, nutrition education, hot meals (breakfast, lunch, and dinner from Monday to Friday), counseling, support groups, recreational activities, and access to medical, medications, and support services.
Assists individuals who have HIV/AIDS or other chronic illnesses in receiving housing, medical services and medication needs, receive social services regarding food, arranging transportation.
Provides 40 housing units with supportive and permanant housing to individuals and families that are homeless and have a disability, including those with a severe and persistent mental illness, chronic illness or substance use, or physical impairments. All residents receive case management services to assist with an independent lifestyle.
Provides affordable housing and community based supports to homeless and low income individuals as well as families who have been diagnosed with HIV/AIDS. Offers components aimed at providing individuals with the supports needed to remain in stable housing in the community as they struggle with their illness.
