Provides case management services to adults with chronic health conditions, serious mental illness, or HIV/AIDS to assist them in staying healthy, out of inpatient care, hospitals, and emergency rooms.
Assists in identifying appropriate services, linkage and referral to recovery-oriented services and other services in accordance with goals identified by the recipient in a written case management plan.
Provides placement referrals to those people 18 years of age and older, who are mentally and/or physically impaired, in need of a supervised living situation, and who meet specified income levels. Assists clients to access a family-type adult home, an adult home, or a nursing home placement.

Provides recreational and creative arts activities in a highly supervised after-school setting for youth ages 7 to 12 years with mental health needs. Offers a range of small group activities, such as gym, recreation, music, and arts and crafts as well as homework assistance. Provides transportation to and from the program.
Offers an after school programs for children with the goal to support the child and enhance positive self-esteem. Provides an after school program for any child Gloversville Middle School, Kingsborough Elementary School and Park Terrace Elementary School in Gloversville, Fonda-Fultonville Elementary and Middle Schools.
Offers assistance to individuals with disabilities with job preparation, development, training, and coaching within the community. Services also include travel training, assessment, and orientation.
Offers a blended prevocational and day program that provides employment opportunities through training and specializes in hand assembly projects for businesses and organizations. Individuals enrolled may also learn daily living, social and recreational skills that increase their self-esteem and work ethic. Opportunities program encompasses day prevocational training, job coaching for community employment and volunteer positions.
Provides an alcohol and drug abuse rehabilitation program, a 28 day inpatient treatment program, job training, and clean and sober housing. There is a waiting list for this service.
