NY Connects is a trusted community resource that provides free information and assistance needed to make informed decisions about long-term care. Long-term Care can include many services that will help individuals remain independent in their daily lives.
Provide living units for individuals with very low, low, and moderate incomes who are ages 62 years and older or have some type of disability.
Provide home delivered meals to individuals recently discharged from the hospital in an effort to reduce hospital readmission for an average of 14 to 30 days of services.
Works to addresses the patient’s rights issues for seniors when there are patient-right related problems, such as being denied admission to a hospital, an unsafe transfer, premature discharge from a hospital, poor discharge planning, poor quality of care, or lack of post-hospitalization care.

Funding can be used towards heating systems repairs or replacement, electrical repairs or updates of electrical systems, plumbing repairs, structural repairs, and accessibility measures.
Offers home rehabilitation assistance for low-to-moderate-income older adults with qualifying repairs.
