Care Coordination
Provides care coordination services for adults, children and families. Supports include scheduling doctor appointments, help with arranging transportation, medication management, access to specialists & providers, support system engagement, and connecting with educational supports.
Program Information
Eligibility
Individuals must:
* currently have active Medicaid
* have 2 or more chronic health conditions, OR be living with HIV/AIDS, OR have a serious mental illness
