Care Consultations

Provides care consultations to families living with Alzheimer's disease. Offers one-on-one support from a trained specialist who is able to discuss the progression and unique aspects of the disease, lay out the various decision points the family will be facing during this journey and provide the family with community-based support resources as the family works its way through those decisions.
