Care Management
Provides patients with assistance regarding short-term needs such as physical therapy, occupational, and respiratory therapies, IV antibiotics, chemotherapy, those with complex medical needs, individuals with a physical or cognitive impairment, or those at risk for repeat hospitalizations. Services offered include home health aide and personal care services, nursing, nutritional services, medical social services, speech and language pathology, and medical supplies and equipment.
