Home and Community Based Services Waiver Program (HCBS)
Assists families with individualized in-home care coordination services for youth ages 5 -18 years of age with mental health challenges. Services include intensive in-home services, crisis response, respite services, family support, information and referral, family peer advocacy, and skill building. All service plans are developed with the participant, parent-caretaker/ legal guardian, individualized care coordinator, and other supporters identified by the child and family.
Children ages 5-21 who have a diagnosed serious emotional disturbance and who are at risk for placement out of the home

Applications may be completed by a parent/guardian, primary clinical program, ICM, probation, school district staff, private therapist, or other service provider
