The Expanded In-Home Services for the Elderly Program (EISEP) provides housekeeping and personal care services, respite services for family members of EISEP clients, ancillary services and case management services. Services are provided under contract through licensed home care agencies.
Provides non-medical companion service for individuals in the community requiring or requesting a continuous or intermittent companion for safety, and socialization. Services include transportation, meal preparation, and shopping. Provides at-home medical care for individuals, including medication management and assistance, nutrition, nursing, and personal care assistance.
Serves as a focal point for older adults in the community which offers a wide variety of services and activities. Provides several activities including guest speakers, numerous exercise classes, card playing, bus trips, sport activities, and a daily lunch program.
Provides a day program, following a medical model, for individuals ages 65 years and older to receive all the services of the nursing home during the day and sleep in their own beds at night. Offers daily activities to encourage independence at home, personal, and nursing care.
Provides home-based care for individuals who are ill, elderly or convalescing. Provides assistance with total care, short-term care, meal preparation, shopping and errands, light housekeeping, and respite care. Assists with daily living activities such as bathing, dressing and toileting, and health-related care, including monitoring vital signs and changing bandages.
Provides services to assist adults age 18 and older who, because of physical or mental impairments, can no longer provide for their basic needs for food, clothing, shelter or medical care, or protect themselves from neglect, abuse or hazardous situations, and have no one willing and/or able to help in a responsible manner.
Provides assistance to help older adults to remain in their home instead of entering a nursing home or other community residence.
Provides health and wellness programming to seniors. Offers education and travel opportunities, fitness options such as yoga, Tai Chi, low impact aerobics, folk dancing as well as creative arts, and cooking instruction classes.
Provides an adult day care respite center and in-home care program. Respite services include supervised social activities computer lessons, presentations, lunch, showering assist, and laundry assistance. In-home care services include social companionship, housekeeping, help with errands and doctor’s appointments, and medication reminders. Also, provides non-emergency medical transportation.
