The Aging Out Adolescent Program works with the Albany County Department of Mental Health helping adolescents “aging out” of the Children’s Mental Health System make the transition to adult services and live as independently as possible. In addition to counseling and case management, we also help with housing, education, vocational training and employment.

Provides services and support for youth from birth to age 21 years who have a severe chronic illness or physical disability that significantly interferes with normal growth and development.
The Children with Special Healthcare Needs program is a referral linkage program to provide assistance to families for children birth to 21 years old who have serious, chornic, physical, developmental, behavioral, medical or emotional conditions.
Assists with accessing health and non-health care services. Helps with obtaining insurance, educational resources, social program assistance, and medical equipment.
Provides specialized meditation for disputes that include students with disabilities, parents/guardians of children who are receiving early childhood intervention services, agricultural mediation, and conflict coaching to assist individuals with one-on-one strategizing for conflict resolution.
Provides prevention and support services for adolescents ages 12 to 17 years and young adults ages 18 to 21 years, who have or are at risk of developing a substance use disorder.
